We are a 71 Bed Skilled Nursing Facility seeking compassionate and dedicated Certified
Nursing Assistants (CNAs) to join our team.

As a CNA, you will provide exceptional patient care, offering physical support and assisting
with daily living needs. Your role will involve monitoring vital signs, assisting with activities
of daily living, engaging in meaningful interactions with residents, and advocating for their
well-being.

Responsibilities

  • Assist with daily living activities to make sure the patient is taken care of. This includes turning and ambulating patients and delivering food and beverages to patients throughout the day.
  • Help patients with personal hygiene, such as providing bedpans and helping them bathe.
  • Process a wide range of information from other professionals and work with physicians, caregivers, and nurses.
  • Closely monitor and record food intake, as well as urinary and fecal output, to be shared with key medical and nursing staff.

Requirements and skills:

  • High School diploma or GED
  • State Certified Nursing Assistant Certification
  • Current CPR Certification (Preferred not Required)
  • Basic computer skills
  • The ability to think and work independently, with direction from a team leader or manager, while maintaining efficient and consistent communications at all times
  • Must also have excellent oral communication skills when speaking with other health professionals in the presence of residents (i.e. “bedside manner”)
  • Can endure high-pressure situations in a fast-paced, time-sensitive environment and keep a calm, professional demeanor throughout

POSITION: CULINARY SERVICES COOK
RESPONSIBILITY: DIRECTOR OF CULINARY SERVICES

General Summary of Work:

Works under the direction and supervision of the Director of Culinary Services, in the
preparation, serving and storage of meals in accordance with printed menus, standardized
recipes, special dietary requirements and state/federal.and local regulations.

Principle Tasks:

  • Preparation of daily meals according to menu plan, and in compliance with regulatory requirements
  • Able to manage portion control, insuring proper preparation of select foods to meet individual need
  • Demonstrates close attention to detail
  • Demonstrates organizational skills
  • Demonstrates a working knowledge of regulatory requirements as related to food preparation, serving, delivery and storage
  • Demonstrates competency with maintaining kitchen safety and infection control principles
  • Able to communicate appropriately with a diverse group of individuals including
  • residents, family members, facility staff and persons from outside the organization

DAILY FUNCTIONS

  • Prepare meals, nourishments and special dietary requests according to established menu plan
  • Able to read, prepare recipes and maintain portion control
  • Able to read and follow daily production sheets including pull and prep lists
  • Serves food according to established facility guidelines and verifies trays for accuracy and completeness before serving
  • Reads and follows work schedules and job tasks lists
  • Maintains cleanliness of kitchen and equipment
  • Assures food and supplies are stored according to regulatory guidelines and infection control guidclines
  • Properly reads and records refrigerator/freezer temperatures and sanitizer levels
  • Assures nourishments, food and leftover food products are properly labeled and dated
  • Follows food storage system FIFO (First In First Out)
  • Responds appropriately to nursing requests for resident nourishments and substitutions as necessary .
  • Routinely tastes and monitor food flavor, texture, color and temperature prior to service
  • Assists in the delivery of food to residents as needed and as assigned
  • Knows where SDS sheets are located
  • Maintains a work area that is cleaned/sanitized prior to work in assigned area
  • Ensures sanitizing buckets are properly filled at the start of each shift as well as
  • refilled throughout shift to maintain proper level
  • Ensure three compartment sink is filled per protocol and tested with test strips at each
  • filling of the third tank to ensure proper sanitation. Documents read ings per policy
  • Assure safe receiving, storage, preparation and service of food
  • Protect food in all phases or preparation, holding, service, cooking, and transportation using HACCP guidelines
  • Complete cleaning tasks as scheduled or sooner as needed, i.e. all spills are wiped or
  • mopped immediately, tray and transport carts are cleaned and sanitized inside and out after each use
  • Ensure trash is taken out at the end of the shift and as needed in the appropriate manner, maintain cleanliness of garbage cans and lids and ensures dumpster lids are closed after use
  • Floors must be swept and mopped using a clean mop head, water and appropriate chemical usage
  • Ensure proper storage of mop heads and brooms
  • Follows facility dress code and hygiene policy
  • Demonstrates flexibility, cooperativeness, courtesy and teamwork
  • Role models good work habits, punctuality, attendance, appearance, and conduct
  • Observes and follows facility protocols with respect to resident dignity, privacy and confidentiality
  • Maintains a positive and professional demeanor
  • Must not pose a direct to the health or safety of other individuals in the workplace

GENERAL KNOWLEDGE, SKILLS, AND ABILITIES

  • Must be able to read, write and understand the English language

PHYSICAL DEMANDS AND WORKING CONDITIONS

  • Able to push/pull move and/or lift a minimum of 25 Ibs to a minimum height 5 feet and be able to push, pull, move and/or-carry such a weight-a minimum-of-50 feet
  • Able to withstand working in extreme temperatures, hot and cold

QUALIFICATIONS

  • At least one year of applicable experience is preferred
  • Current knowledge of job-related state and federal regulations

  • Serv Safe Certification preferred
  • Must be able to perform the essential functions of the job, with or without reasonable accommodation

Crystal Lake is Seeking RN’s to be a part of our team. We are a 71-bed Skilled Nursing Facility that offers long-term care and short-term rehab to our residents. Full time, Part time, and Per Diem up to 40 hours per week including every other weekend positions available. The RN will be an essential person in our nursing department assisting with the daily management of the residents’ nursing needs and oversight of direct care staff. Responsibilities
